RPM codes in 2026, in plain English

Two new codes changed how short monitoring months are billed. Here is what each RPM code covers and how to avoid the common mistakes.

The PulseCare360 Team,

2026 brought the biggest change to Remote Patient Monitoring billing in years: practices can now be paid for months with fewer than 16 days of readings and for shorter management time.

The device supply codes

99454 still covers 16 or more days of readings in a 30-day period. The new 99445 covers 2 to 15 days, so a patient who misses a week is no longer a lost month.

The management codes

99457 covers the first 20 minutes of treatment management with at least one interactive conversation, and 99458 each additional 20 minutes. The new 99470 covers 10 to 19 minutes.

Common mistakes

  • Counting the same minutes toward RPM and CCM.
  • Missing the interactive communication requirement.
  • Billing RPM and RTM for the same patient.

Our platform tracks days and minutes per program automatically and shows which code each patient qualifies for at month end.
