General
What is PulseCare360?
PulseCare360 helps medical practices run Remote Patient Monitoring, Chronic Care Management, Remote Therapeutic Monitoring and Principal Care Management without adding work for their staff.
We combine a care platform with a clinical team that handles eligibility, enrollment, daily monitoring, follow-up calls and billing preparation.
Which practices do you work with?
Primary care, internal medicine, specialty clinics and multi-provider groups. If you care for patients with hypertension, diabetes, COPD, heart failure or similar conditions, remote care is likely a good fit.
We work across Medicare, Medicaid, commercial plans and veterans programs.
Services & programs
What is Remote Patient Monitoring (RPM)?
RPM uses connected devices — blood pressure cuffs, scales, glucose meters, pulse oximeters — to send readings from home. Our clinical team reviews them, watches trends and talks with patients. Providers are alerted when clinical action is needed.
What is Chronic Care Management (CCM)?
CCM is a CMS-recognized program for patients with two or more chronic conditions expected to last 12 months or more. It covers care coordination, medication management, care plan oversight and communication between providers, performed under the general supervision of the billing provider.
Can one patient be in RPM and CCM in the same month?
Yes, as long as the time for each program is separate. The same minutes cannot count toward both. Our platform keeps time per program automatically.
RPM and RTM cannot be billed together for the same patient.
Do you support PCM and RTM?
Yes. PCM is for patients with one high-risk chronic condition. RTM covers non-physiologic data such as therapy adherence, pain and function. All four programs run on the same platform and team.
How it works
What does onboarding look like?
Five steps: we analyze your census for eligibility, enroll patients (consent, device shipping and setup), monitor daily, flag meaningful changes to providers, and deliver a ready-to-submit billing report each month.
Will this add work for my staff?
No. We act as an extension of your practice and handle patient communication, devices, monitoring and documentation. Time and interactions are recorded as they happen, so there are no timesheets to chase.
Who monitors patients day to day?
A dedicated team of medical assistants, supervised by nurse practitioners, reviews readings, follows symptom trends and supports each patient’s care plan every day.
Billing & revenue
Which CPT codes do you support?
RPM: 99453 (setup), 99454 (device supply, 16+ days), 99445 (device supply, 2–15 days), 99457 (first 20 minutes), 99458 (each additional 20 minutes) and 99470 (10–19 minutes).
CCM: 99490 (first 20 minutes), 99439 (each additional 20 minutes) and 99487 (complex CCM).
We update the platform as CMS rules change each year.
How does billing work?
Every billable interaction is documented in real time. At month end you receive a report by patient and code, ready for your billing team to submit under your practice’s NPI. Reimbursement goes directly to your practice.
Which insurance plans cover RPM and CCM?
Medicare is the main payer. Many commercial plans and some Medicaid programs also cover these services; coverage varies by plan and state. We review your payer mix during the eligibility analysis.
Compliance & consent
Is patient consent required?
Yes. Consent is collected before services begin and filed in the patient’s record. We handle the consent process as part of enrollment.
Who is the billing provider?
The physician or qualified health professional at your practice (MD, DO, NP, PA, CNS or CNM) is always the billing provider. Our clinical staff work under their general supervision.
How is patient data protected?
Access is role-based, sign-ins are protected against guessing, sessions time out and actions are logged. See our Trust center for details, and ask us for our security documentation.
Getting started
Do we have to enroll everyone at once?
No. Start with a small group, see the results and expand at your own pace. There is no minimum enrollment.
Is there a cost to get started?
Talk to us about pricing for your practice size and model. We will show you the numbers during your demo.
Patients
Will this cost me anything?
Your health plan usually pays most of the cost. You may owe a small share called coinsurance, which some plans cover. Your care team will go through your coverage before you start.
What happens if a reading looks unusual?
A member of your clinical team calls you, usually the same day, to talk it through and decide whether your doctor needs to be involved right away.
Do I need Wi-Fi or a smartphone?
No. Most devices send readings over a built-in cellular connection. There is no Wi-Fi to set up and no app to download.
Can I stop at any time?
Yes. Joining is your choice and nobody will enroll you without asking. Tell your care team or your doctor’s office whenever you want to stop.
Does this replace my doctor visits?
No. It supports the visits you already have. Your doctor stays in charge of your care; we help catch changes earlier.
