PulseCare360 (“we”, “us”) respects your privacy. This policy explains how we collect, use, share, keep and protect information when you use our website, patient portal and care programs (RPM, CCM, RTM and PCM).

Contact details such as name, phone, email and address
Health information shared by your provider or recorded during your program, including device readings
Messages, call notes and check-in answers
Basic technical information such as IP address and browser type

To deliver and coordinate your care program
To send reminders and updates you agreed to receive
To respond to questions and improve our services
To meet legal and contractual obligations

4. Sharing

We share information with your healthcare provider, clinicians working under your care plan, vendors bound by business associate agreements, and authorities when the law requires. We do not sell personal or health information. Text-messaging opt-in data is never shared with third parties for marketing.

5. Your rights

You may request access, corrections or deletion (subject to legal record-keeping rules), opt out of non-essential messages, and leave your program at any time.

6. Retention

Medical and billing records are kept for the periods required by law. Other information is deleted or de-identified when no longer needed.

7. Security

We use encryption in transit, role-based access, sign-in protection and activity logging. No system is perfectly secure; tell us right away if you suspect a problem.

8. Children

Our services are for adults. We delete information collected from children under 13 without verified parental consent.
