Remote Therapeutic Monitoring in plain terms

What therapeutic monitoring means for you

This program follows how a treatment is working for you — exercises for a sore knee or back, or an inhaler for your breathing.

Your team checks your progress: are you doing the exercises, is the pain easing, are you breathing easier? You answer a few short questions or use a simple device at home.

What a month with us looks like

No dashboards to check and no alerts to decode. One small step is yours; the rest is ours.

  1. Your partDuring the month

    You check in

For therapeutic monitoring you tell us how therapy is going — exercises, medication use, symptoms or pain level, whatever your plan asks for.

  2. Handled for youWithin the week

    Your care team reviews it

Someone on our team checks your update against the plan your doctor set. Most months everything is on track.

  3. Handled for youIf something looks off

    We reach out

A team member calls to check in, often before you would have noticed anything. Your doctor is brought in when needed.

  4. Handled for youEvery month

Your doctor sees the whole picture

Your progress over weeks — not a single moment — is ready for your next visit.

Nothing new to learn

  • No smartphone needed

Most devices send readings over their own cellular connection. No Wi-Fi? We will set you up another way.

  • Delivered and set up for you

Your device arrives ready. We stay on the phone with you, step by step, until it feels easy.

  • A person picks up

Questions about a reading, a device or your plan go to a real clinical team member — not a menu.

  • Your information is handled under HIPAA
  • Support available 24/7
  • Your own doctor stays in charge

People, not readings

A few moments from our care calls, shared with permission and with details changed for privacy.

“Someone actually asked how I was doing.”

A routine monthly call turned into a long conversation about retirement, grandchildren and feeling invisible. She told her nurse it was the first time a health call felt like it was about her, not her chart.

Sometimes care is simply making sure nobody carries things alone.

“What were my numbers this week?”

After a long hospital stay, one patient was unsure he could manage his heart and kidney conditions at home. A few weeks into monitoring he was calling us first to ask about his readings.

The right support turns numbers into ownership.

A same-day call that mattered

During a check-in, a patient described symptoms that did not sound right. Her care team arranged urgent care that afternoon. Months later she was cooking, driving and back at church.

An early call can mean getting your life back.

Room to say “I’m not okay”

One patient kept saying he was fine. His care team noticed the pause before the words and gave him space to talk. Together they found local support — and a path back to finishing his education.

Care should make room for the whole person.

Readings for two

A husband caring for his wife kept forgetting his own blood pressure checks. His nurse suggested they take readings together each morning. They now call it their coffee date.

Caregivers need care too.

A promise kept

After his first heart attack, one patient promised himself he would pay attention. Daily readings and a monthly call helped him keep that promise, one morning at a time.

Tools help. People help you keep going.

Questions patients ask us

Will this cost me anything?

Your health plan usually pays most of the cost. You may owe a small share called coinsurance, which some plans cover. Your care team will go through your coverage before you start.

What happens if a reading looks unusual?

A member of your clinical team calls you, usually the same day, to talk it through and decide whether your doctor needs to be involved right away.

Do I need Wi-Fi or a smartphone?

No. Most devices send readings over a built-in cellular connection. There is no Wi-Fi to set up and no app to download.

Can I stop at any time?

Yes. Joining is your choice and nobody will enroll you without asking. Tell your care team or your doctor’s office whenever you want to stop.

Does this replace my doctor visits?

No. It supports the visits you already have. Your doctor stays in charge of your care; we help catch changes earlier.

Remote monitoring is not emergency care. In a medical emergency, call 911.

Questions before you start?

A member of your clinical team will answer — the same voice each time.
